The following service agreement is for _________ hours of service at a cost of _________.

A deposit amount of _________  is required to book this date with a balance of _________

being due in full on _________ at the conclusion of the service.

Any additional service being requested beyond the hours specified above will be at the discretion 

of the DJ working at the event. Agreements for additional service will be set at a rate

established by the DJ at the event and full payment for extra time is required to be made

at the conclusion of the service.

Once the service agreement has been completed and deposit received, the above requested date

will be reserved for your event.  Should a cancellation of service be received prior to 60 days

from the date requested, 1/2 of any payments or deposits will be refunded.  Any cancellations

made within less than 60 days will result in the forfeit of the full amount of any payments or deposits. 

Back Beat Music Service retains the right to refuse service before or during the time

reserved should the environmental conditions or conduct of guests become unreasonable

or detrimental to the equipment or personnel of Back Beat Music Service.
